Early Mobilization
Begin within 24h of surgery when stable. Early ambulation reduces DVT, pneumonia, deconditioning, ileus. Assess pain control before mobilizing.
Progressive Ambulation
Progress from dangling to standing to walking. Use assistive devices as needed. Monitor vitals before, during, after activity. Increase distance daily.
Discharge Planning
Assess home for barriers (stairs, bathroom). Arrange home health, outpatient PT, or skilled rehab. Educate on home exercise program and warning signs.
Important Notes
- Hip precautions (posterior): no flexion >90ยฐ, no adduction past midline, no internal rotation.
- Assess for orthostatic hypotension before ambulating โ common after bed rest.
Clinical Tips
- Use gait belt for all transfers to protect patient and nurse.
- Splint or brace surgical wounds for support during ambulation.
Summary
Post-surgical rehabilitation emphasizes early mobilization, progressive ambulation, and safe discharge planning.
